CVS Caremark Charitable Trust Grantee Spotlight: Wesley Health Center
Arizona’s Wesley Health Center is taking a stand against chronic disease. The health center has put together a comprehensive disease management program that is helping hundreds of patients manage their chronic conditions. Thanks to a partnership with CVS Caremark and the National Association of Community Health Centers (NACHC), Wesley Health Center has formed an Integrated Chronic Disease Multi-disciplinary Team (ICDT) that is working one-on-one with patients who have hypertension and diabetes.

The CVS Caremark Charitable Trust is partnering with the National Association of Community Health Centers (NACHC) and its members to find innovative solutions to chronic disease management. “Innovations in Community Health” grants, totaling more than $1 million, have been awarded to 21 community health centers to support the development of programs that focus on the treatment and management of chronic illnesses. Community health centers provide affordable health care services to more than 22 million patients at more than 9,000 locations across the country.
